What to include

  • The URL of the listing
  • The information you believe is inaccurate or outdated
  • The correct information
  • A reliable public source or supporting document, when available

Do not send treatment records, private health information or other sensitive personal information.

Mindwell Find may review available sources, contact relevant parties or request additional support before changing a listing. Submitting a request does not guarantee that a requested change will be made.
